SJ Rehab provides specialist case management for adults, children and young people following serious and complex injury.
Our approach is clinically led and focused on understanding the individual, identifying rehabilitation priorities and coordinating the right intervention at the right time. We work collaboratively with clients, families, solicitors, deputies, insurers and treating professionals to keep rehabilitation purposeful, proportionate and focused on meaningful outcomes.
Experienced Case Management
SJ Rehab is led by Sam Jobling, an experienced Registered Nurse, IRCM Registered Case Manager and Advanced Registered Practitioner Member of BABICM, with support from a Case Manager Assistant and dedicated administrative support.

We provide Initial Needs Assessments and ongoing rehabilitation case management for adults, children and young people with serious and complex injuries, including:

Professional Support for Case Managers
Alongside our rehabilitation case management services, SJ Rehab provides one-to-one professional supervision, Clinical Sense Checks and peer group supervision for independent case managers.
