Case Management for Serious & Complex Injury

Following a serious or life-changing injury, rehabilitation can involve medical treatment, therapy, equipment, care and support from a number of different organisations. SJ Rehab helps clients and families understand the available options, identify appropriate rehabilitation priorities and coordinate the professionals involved.

We support adults, children and young people following serious and complex injury, including:

• Acquired and traumatic brain injury 
• Birth injury, including cerebral palsy 
• Multiple traumatic injury 
• Amputation and complex orthopaedic injury

• Complex psychological and behavioural presentations 

We recognise that serious injury can also affect cognition, emotions, behaviour, mental health, learning and neurodevelopment. These needs can be considered as part of the wider rehabilitation programme.

We take a person-centred approach, ensuring that you or your loved one is always at the heart of every decision. Our role is to:

• Assess your needs through a comprehensive Initial Needs Assessment (INA)
• Develop a tailored rehabilitation plan that reflects your goals and values
• Coordinate the right professionals, therapists, and support workers
• Support and monitor progress continuously — adapting as things change
• Help make sure your views are heard, liaising with legal, medical, therapy and care professionals involved in your rehabilitation
