Rehabilitation in Practice

How We Work

At SJ Rehab, we believe that effective rehabilitation starts with understanding the individual, not just the injury. Every recommendation is informed by clinical reasoning, collaboration and meaningful goals that reflect the person's circumstances.


This page explains the principles that guide our work through practical examples of rehabilitation case management. The examples are based on common themes encountered in practice and have been deliberately kept general to protect client confidentiality. They are intended to help clients, families, solicitors and healthcare professionals understand how rehabilitation decisions are approached in practice.

🧩 Coordinating Complex Rehabilitation

The Challenge

Individuals with complex injuries often require input from a wide range of professionals, including therapists, medical specialists, support workers, schools, employers, solicitors and deputies. Whilst each professional has an important role, rehabilitation can become fragmented if communication and planning are not coordinated effectively.


Without a shared understanding of the individual's goals, rehabilitation may become disjointed, with duplicated interventions, conflicting advice or unnecessary demands placed on the individual and their family.


Examples of Our Work

Examples of how we have coordinated complex rehabilitation include:

  • Bringing together multidisciplinary teams to establish shared rehabilitation goals and improve communication between professionals. 
  • Coordinating rehabilitation programmes involving physiotherapy, occupational therapy, speech and language therapy, neuropsychology and support workers to ensure interventions complement one another. 
  • Liaising with NHS and independent providers to help maintain continuity of rehabilitation where services overlap or change. 
  • Supporting communication between families, treating professionals, solicitors, deputies and support providers to ensure everyone remains informed and working towards the same outcomes. 
  • Reviewing rehabilitation recommendations as needs evolve, ensuring interventions remain appropriate, proportionate and focused on meaningful outcomes. 
  • Coordinating equipment provision, accommodation adaptations and community services to support safe discharge and maximise independence. 
  • Organising and chairing multidisciplinary meetings to review progress, address challenges and agree future rehabilitation priorities. 
  • Identifying gaps in rehabilitation provision and making evidence-informed recommendations to address unmet needs.


Our Approach

Effective rehabilitation is rarely achieved by one professional working in isolation. We believe the best outcomes come from collaborative working, clear communication and regularly reviewing progress to ensure rehabilitation remains person-centred and responsive to changing needs.


Our role is to coordinate the rehabilitation journey, helping to bring together different professional perspectives whilst keeping the individual's goals at the centre of every decision.

👥 Supporting Support Worker Packages

The Challenge

A well-designed support worker package provides more than practical assistance. When integrated effectively within the wider rehabilitation programme, support workers can play an important role in promoting independence, reinforcing therapeutic strategies and supporting individuals to achieve their rehabilitation goals.


Achieving this requires close collaboration between the support provider, case manager, therapists, the individual and their family.


Examples of Our Work

Examples of how SJ Rehab supports support worker packages include:

  • Working alongside trusted support providers to help develop support worker packages that reflect the individual's assessed needs and rehabilitation goals. 
  • Collaborating with therapists to ensure support workers understand rehabilitation strategies and can reinforce these during everyday activities where appropriate. 
  • Coordinating communication between the support provider, multidisciplinary team, family and legal representatives to ensure everyone is working towards shared goals. 
  • Monitoring how the support package is contributing to the individual's rehabilitation and making recommendations where changes may improve outcomes. 
  • Supporting regular reviews of the support package as the individual's needs, abilities and goals change over time. 
  • Working collaboratively with partner organisations to ensure the support package remains responsive to the individual's changing circumstances and wider rehabilitation programme.


Our Approach

SJ Rehab works in partnership with experienced organisations that are responsible for the recruitment, employment and day-to-day management of support workers.


Our role is to ensure the support worker package is fully integrated within the individual's wider rehabilitation programme. By working collaboratively with support providers, therapists, families and legal representatives, we help ensure support workers understand the individual's goals and contribute to promoting independence, participation and meaningful rehabilitation outcomes.


By working in partnership with specialist support providers, we are able to remain focused on our independent rehabilitation case management role whilst ensuring clients benefit from experienced organisations that manage the operational delivery of support worker packages.

🏫 Supporting Education and Transitions

The Challenge

Returning to, or remaining in, education following a serious injury or neurological condition can present a range of challenges. Difficulties with fatigue, attention, emotional regulation, communication or physical access can affect participation, even when these challenges are not immediately visible.


Supporting a child or young person in education requires collaboration between the individual, family, school and wider multidisciplinary team to ensure the right support is in place. group therapy, and medication management.


Examples of Our Work

Examples of how SJ Rehab has supported education include:

  • Working collaboratively with schools, families and treating professionals to identify barriers to learning and participation. 
  • Coordinating communication between education settings and the multidisciplinary team to ensure rehabilitation goals are understood and appropriately supported. 
  • Providing recommendations to support successful transitions between educational settings. 
  • Supporting discussions around reasonable adjustments to promote participation, independence and wellbeing within the school environment. 
  • Liaising with education professionals to ensure therapeutic recommendations are considered within the school day where appropriate. 
  • Monitoring progress and adapting recommendations as educational needs and rehabilitation goals evolve. 
  • Supporting families to navigate the education system and access appropriate services where additional support may be required.

Education is about far more than academic achievement. It provides opportunities for independence, confidence, friendships and participation in everyday life.


Our role is to work collaboratively with families, schools and professionals to ensure educational support reflects the individual's rehabilitation needs whilst promoting meaningful participation and long-term outcomes.

🏛 Working with Statutory Services

The Challenge

Rehabilitation often extends beyond healthcare. Individuals and families may require support from local authorities, education providers, NHS services, housing teams and community organisations. 


Understanding how these services fit together can be challenging, particularly when multiple agencies are involved.safety assessments to help seniors stay active and healthy.


Examples of Our Work

Examples of how SJ Rehab has worked alongside statutory services include:

  • Liaising with NHS teams to coordinate ongoing rehabilitation and facilitate communication between statutory and independent providers. 
  • Working collaboratively with local authority professionals to support access to appropriate community services. 
  • Coordinating communication between health, education and social care professionals to promote a joined-up approach to rehabilitation. 
  • Supporting referrals for specialist assessments and statutory services where additional needs have been identified. 
  • Working alongside families to help them understand available services and navigate complex health, education and social care systems. 
  • Collaborating with statutory services to support safe discharge planning and community rehabilitation. 
  • Identifying gaps in service provision and making recommendations to support the individual's longer-term rehabilitation goals.

By working collaboratively with statutory providers alongside independent professionals, we help ensure individuals receive coordinated, person-centred support that reflects both their rehabilitation goals and the services available to them.

🛡 Safeguarding and Risk Management

The Challenge

Individuals living with the effects of serious injury or neurological conditions may be at increased risk of abuse, neglect, exploitation or self-neglect. Safeguarding concerns are not always immediately obvious and often develop gradually through changes in behaviour, support arrangements or living circumstances.


Recognising concerns early and responding appropriately is essential to protecting an individual's wellbeing whilst promoting their rights, independence and autonomy.


Examples of Our Work

Examples of how SJ Rehab has supported individuals where safeguarding concerns have arisen include:

  • Identifying safeguarding concerns through regular case management visits and ongoing communication with the individual, family and wider multidisciplinary team. 
  • Working collaboratively with local authority safeguarding teams, healthcare professionals, solicitors and deputies to ensure concerns were managed appropriately. 
  • Coordinating multidisciplinary meetings where safeguarding concerns formed part of wider rehabilitation planning. 
  • Reviewing support arrangements and making recommendations where changes were required to improve safety and reduce identified risks. 
  • Supporting individuals to access appropriate statutory services and specialist agencies where additional intervention was required. 
  • Maintaining detailed documentation and clear communication to ensure safeguarding concerns were appropriately shared and managed in line with professional responsibilities. 
  • Balancing an individual's right to make informed decisions with the need to reduce the risk of harm, recognising the importance of least restrictive practice.


Our Approach

Safeguarding is not a separate part of rehabilitation; it is an integral part of good case management.

We recognise that effective safeguarding requires professional curiosity, ongoing assessment and collaborative working. Our role is to identify concerns, coordinate appropriate responses and work alongside statutory agencies and the wider multidisciplinary team to promote safety whilst respecting the individual's rights, wishes and rehabilitation goals.

The examples above illustrate common themes encountered in rehabilitation case management. Every individual has unique goals, challenges and priorities, which is why rehabilitation should always be tailored to the person rather than following a standard pathway.
