Serious & Complex Injury Case Management

SJ Rehab provides independent, clinically led rehabilitation case management for adults, children and young people following serious and complex injury.

We accept referrals from solicitors, deputies, insurers and other professionals, providing Initial Needs Assessments and ongoing case management for clients with brain injury, birth injury, amputation, complex orthopaedic and neurological injury, and complex psychological and behavioural presentations.

SJ Rehab is led by Sam Jobling, Registered Nurse, IRCM Registered Case Manager and Advanced Registered Practitioner Member of BABICM, with extensive clinical and rehabilitation case management experience.

We understand the clinical, legal and professional frameworks that shape rehabilitation following serious and complex injury.

We are experienced in supporting adults, children and young people following:

• Brain injury, including acquired and traumatic brain injury 
• Birth injury, including cerebral palsy 
• Multiple traumatic injury

We recognise that serious injury may also involve cognitive, emotional, behavioural, neurodevelopmental and mental health needs, which are considered as part of the wider rehabilitation picture.

• Initial Needs Assessments (INAs) - Comprehensive, timely reports with recommendations.

• Ongoing Case Management - Coordinating multidisciplinary rehabilitation including therapy, equipment, care and support, education, vocational rehabilitation and community participation according to individual need.

• Care and Support Package Coordination – Working with appropriately registered care providers and other professionals to establish, coordinate and review support packages according to the client's needs.

• Liaison and Communication - Acting as a central point of contact for families, solicitors, and clinicians.

• Capacity, Safeguarding, and Decision-Making Support - Support for complex decision-making, in collaboration with legal professionals and MDTs.

We work across litigation, statutory and privately funded rehabilitation.

Clinically led – Case management is led by an experienced Registered Nurse and IRCM Registered Case Manager.

Professionally accountable – Practice is informed by IRCM standards and Advanced BABICM membership.

Independent and objective – Recommendations are based on identified rehabilitation needs and clinical reasoning.

Responsive – Clear communication with clients, families, litigation teams, deputies and the wider MDT.

Proportionate – Rehabilitation recommendations are kept under review so intervention remains purposeful and appropriate to the client's changing needs.

Personal service – As an independent case management company, we provide continuity and senior clinical oversight rather than a high-volume model.

SJ Rehab is based in Lichfield, Staffordshire, providing rehabilitation case management across the Midlands and accepting suitable instructions throughout England and the wider UK.
