
Rehabilitation case management helps people following serious or complex injury to access and coordinate the rehabilitation, treatment, equipment, care and support they may need.
A case manager works with the individual, their family and the professionals involved to understand their needs, identify rehabilitation priorities and develop a coordinated plan. As needs change, the case manager monitors progress, reviews recommendations and helps ensure that rehabilitation remains appropriate and focused on meaningful outcomes.
At SJ Rehab, case management is clinically led and person centred. We support adults, children and young people following serious and complex injury, working collaboratively with families, treating professionals, solicitors, deputies, insurers and other organisations involved in their rehabilitation.
