Rehabilitation Case Management for Brain and Catastrophic Injuries

What is Rehabilitation Case Management?

Rehabilitation case management is a process designed to facilitate recovery after a catastrophic injury. According to BABICM (British Association of Brain Injury and Complex Case Management), it involves the coordination of healthcare services, assessments, interventions, and long-term plans tailored to a person’s needs. A rehabilitation case manager is responsible for overseeing all aspects of a client's recovery, from medical treatments to emotional and social support.

As an Advanced BABICM practitioner and Registered Case Manager, Sam Jobling follows the best practices for case management, ensuring that each client receives comprehensive care and advocacy throughout their rehabilitation journey. The key responsibilities of a case manager include:

  • Initial Needs Assessments: Identifying all medical, psychological, and social needs associated with the injury.
  • Developing a Tailored Plan: Creating and implementing a rehabilitation plan that is unique to each individual, focusing on their specific needs and goals.
  • Coordinating Healthcare Services: Ensuring all relevant specialists, such as doctors, therapists, and home care providers, are involved in the rehabilitation process.
  • Monitoring and Advocacy: Ongoing evaluation of progress and making adjustments to the care plan, as well as advocating for the client’s best interests with insurers, healthcare providers, and legal teams.
